The inquiry form
The inquiry form asks for a parent or guardian name, email, optional phone number, service interest, and optional message. Milestones LLC uses its inquiry system to review the request and respond. Please do not include medical records or other sensitive student details.
Contact requests
Sending the inquiry form or reaching out by email or phone is a contact request. It does not imply marketing consent, and it is not an approved booking or confirmed enrollment. The Annex team reviews student fit and available space before an appointment is added to the calendar.
